Using the chart created in your Week 5 Assignment, select what you believe to be the most useful approaches for investigating the events you have identified. Specifically, how would you, as the Assistant to the Regional Director of Quality and Patient Care, use these quality tools as essential resources to develop and to oversee an investigation into the causes of a sentinel event, an adverse outcome, and a never event?  If you believe more than one tool or approach should be used then provide an illustration.  For example, if you believe a fishbone diagram would be useful, replicate a diagram.  You do not need to fill in the diagram, that step is reserved for your Week 7 Signature Assignment.
