Review the draft of the problem statement you developed in Module 1. (A workplace problem in the National Salvage Unit at Progressive Insurance is the processes in place to obtain documents in a timely manner to sell the vehicles at auction. File owners first start a claim and send the lienholders payments and needed documents to release the lien and title on the vehicles that are salvaged. The file owner must obtain a letter of guarantee before sending the payment and this is sometimes not even followed causing problems as well. The letter of guarantee states the amount agreed between both parties and the signed promise to release lien and title to progressive for sale. At this time the file owner should send the address of the specific vendor the vehicle is held at for auction.  In most cases this does not happen, or the incorrect address is provided, and the title / lien is often lost or never released. After the claim is set up it is then sent to a Processor to finish setting up the claim and obtain the remaining needed documents for sale. The processor then monitors to ensure the title is received and sets another team called lien release on the claim to monitor the status of title and reach out to lienholders to receive the titles if not received in four weeks from the payment. 

 

The file owners not following the correct procedure and sending the correct information to the lien holder when the payment is issued causes a rippling effect on the salvage department. If the file owner issues a payment without a signed letter of guarantee the lienholder can opt not to send the needed lien release or title at all or argue that there is still a balance on the account. If a letter of guarantee is obtained and payment is issued without a clear address to send the title or the wrong one the liens are often never released or mailed to the incorrect places and ultimately lost. When a title is lost the salvage department then needs to get a specific lien release from the lien holder that fits that’s states guidelines and then pay to order a duplicate title from the state. The file owners not following the procedures from the beginning of the claim causes the department to have a whole team of 25 people or more to just call lien holders every day to correct the addresses, provide addresses and or order lien releases from the lien holders. Have a whole team they must pay for to correct the mistake of the file owners. This also is not cost-efficient paying thousands of dollars to employ a whole team to look for titles and lien releases but also the costs to the DMVs to order duplicate titles. This is also very time consuming, for the processing team, lien release team and delays the sale of the vehicle at auction. 

The problem here is the file owners are not following procedures when starting a total loss claim causing an extra workload on other departments and a delay in sale. )

Why Ask Why: Review the original problem, ask why again, and then answer that question. Keep on asking “why” again as necessary. What’s at the bottom of the problem, underlying everything?

Conclusion: 
Final problem statement: Restate your problem in a final form and in one sentence. Phrase the problem as a “How…” question (as in “How can we improve quality?” or “How can we reduce patient waiting times in the doctor’s office?”).
Clarification of changes: Note whether or not the changes in your problem statement are based on the application of the restatement process. If your problem statement did change, justify why and how you changed it.
Newly discovered problem knowledge: State what you discovered through the problem restatement process that may require further investigation or research.
